Instructions

– Please complete the form below at least 30 days before the requested field trip date.
– Attach a copy of the invoice or an email confirmation from the event. Make sure that the attached document includes the date and cost of the trip.
– Payment will be returned to the contact person, who will be responsible for the check unless other arrangements have been made.
